The aim of all our support and accommodation provisions is to ensure that the right level of service is provided at all times. Through on-going assessment, monitoring, review and consultation, the service provision will adapt with the changing needs of the Service User.

Through constant research, professionals meetings, membership of specialist bodies, on-going training and Service User involvement, we aim to ensure our service and accommodation provisions are compliant, meet our Service Users' needs, provide realistic expectations and achieve positive outcomes.

   
Supported Housing
   
 

This service provides accommodation and support services for two specific service user groups; a) young adults aged 18-24, including those leaving care, asylum seekers and vulnerable families and b) vulnerable adults aged 18-64 who are rough sleepers, homeless or at risk of facing homelessness. Following a comprehensive needs and risk assessment and the preparation of a support plan, the objective of the service is the progression of the service user to “move on” and live independently.

Our in-house support worker/out reach team focus on matters such as life skills, identifying and sourcing education/training, accessing health and benefits and are available 24 hours a day, including weekends.

Monitored Accommodation
   
 

Monitored accommodation and support services are provided for individuals aged 18 years old onwards, with a variety of needs. The support and accommodation services are usually provided on two levels, but are flexible and adaptable to meet the service users needs:

Low Support Service: This accommodation is regularly monitored by a floating team. The monitoring team will undertake planned, routine and reactive visits several times each week, including out of hours and weekends. Each Service User receives up to two hours of 1:1 key work support each week


Medium Support Service: This level of service provides a greater and more structured staffing presence within the accommodation, including in the evening and weekends. Each Service User usually receives a minimum of two hours of 1:1 key work support each week.

Staffed Accommodation
   
 

Staff are on site 24/7 with support hours available as required and assessed, including floating support for off site activities. Working with a number of local authorities, charities and CIC’s and housing associations, accommodation and support services can be tailored to fit the individual person's needs, based on the referral information, assessment and person centred planning process.

With over 40 members of experienced staff and managers, we are able to support service users with a variety of needs, as assessed and agreed.

Personalisation and Care Management
   
 

For some care services, the Government is leaning towards self directed care, where the Service User receives and controls their own care budget, so that they can put together their own care package and make their own budget choices.

For some people, this is a major change to the way their care services will be managed and financed and our specialist team is able to provide assistance, advice and guidance to help identify and source the most effective, relevant and cost efficient support and care required. Our team can manage the whole process and also offer more specific support for a particular service, as required.

Temporary & Long Term Housing Solutions
   
 

The range of accommodation provided includes both shared and self contained properties and is available throughout the North London area, predominantly in Enfield, Haringey, Barnet, Waltham Forest, Hertfordshire and Essex.

To support our tenants and service users, we provide a 24 hour property management and maintenance service and a wide range of specialist housing related and tenancy sustainment support services.

Move On Services
   
 

Independent living is an aim of the majority of Service Users, to varying degrees. Our support team will assess all Service Users and make this a focal point of the support plan. Our specialist property services team are able to source a wide range of accommodation, to suit the needs of most Service Users, with support levels adjusted and gradually withdrawn, as appropriate.

The support and property teams include a wide range of staff, able to address the needs of most people. Projects can be set up for individuals or groups of people and we have a varied portfolio of properties available, many of which can be altered to suit specific Service Users needs.

Floating Support
   
 

A floating support is available and provided to eligible people, as required and agreed. The support team is independently managed and works in partnership with the housing team/provider to meet the service user’s needs and achieve positive outcomes, with the overall aim to progress the service user to “move on” and live independently, as assessed and agreed.
